Complete this form for each festival your unit is participating in no later than 24 hours prior to the festival.

The information on this form will be used for the purpose of officially declaring your number of marching members and announcing your unit at festivals. Your number of marching members MUST be disclosed and must match the number of performing members on the floor; otherwise your unit may be penalized or disqualified.

This form must be received before your unit will be allowed to perform.
